COMPLETION OF YOUR DEGREE AT USU

Checklist

• Make sure your committee has submitted the Record of Examination Completion form to the Graduate School
• Check your transcript and make sure all incomplete research credits have been changed to a passing grade
• Make all changes required by your committee to your thesis/dissertation
• Carefully review the Publication Guide for Graduate Students and make sure your thesis/dissertation is formatted properly
• Complete the Thesis/Dissertation Format and Style form
• Submit your corrected thesis/dissertation, the Thesis/Dissertation Format and Style form, and a complete copy of a sample article from your journal (for style) to your faculty advisor
• After your advisor has signed off on your thesis/dissertation, submit the corrected version, the form, and journal article to the School of Graduate Studies for review
• Submit your thesis/dissertation for binding, pay the fees, submit the receipt to the School of Graduate Studies
• Complete a Graduation Fee Payment form – pay the $15 fee at the Registrar’s office
• Complete a Commencement Data Card, a Survey of Earned Doctorates (if a doctoral student) and an Alumni Card to the School of Graduate Studies
• Provide the Department with a forwarding address and contact information
• Keep in touch with us – contact the Department occasionally with current information on your progress to include in our newsletter and on our website.  Feel free to set up a graduate fellowship, travel fund, or scholarship to assist the new graduate students after you become a world renowned scholar and alumni of USU
